To be eligible for COMMCARE Services:

Philosophy:


  • Must have a diagnosis of Traumatic Brain Injury (TBI) as the rimary diagnosis on the MA-51 (Medial Evaluation) form
  • Must be between the ages of 21 and 59
  • Pennsylvania Resident
  • Must be Nursing Facility Eligible

Must have substantial functional limitations in 3 or more of the following major life activities:

  • Mobility
  • Communication
  • Independent Living
  • Self-Care
  • Behavior
  • Self-Direction
  • Cognitive Capacity
  • Directed and controlled to the maximum extent as possible by the consumer
  • Meet Health and Safety needs of the consumer
  • Conform with Federal and State requirements
  • Be cost effective
