Autism Waiver

Who is eligible? Pennsylvania residents age 21 and older who meet the medical assistance program clinical and financial eligibility for Intermediate Care Facility for Persons with Other Related Conditions (ICF/ORC) or Intermediate Care Facility for persons with Mental Retardation (ICF/MR) services. The disability must result in substantial functional limitation in three or more of the following areas of major life activity: self care, receptive and expressive language, learning, mobility, self direction, capacity for independent living and economic self-sufficiency.

Unlimited Care Providers, a subsidiary of Choices People Supporting People, was established in 2004 to coordinate services for individuals diagnosed with Traumatic brain injury (TBI) through the COMMCARE waiver program.

The COMMCARE waiver is a home and community based program developed for individuals with a medically determined diagnosis of Traumatic brain injury (TBI). Traumatic brain injury is defined as a sudden insult to the brain or its coverings, not of a degenerative, congenital or post-operative nature, which is expected to last indefinitely and require a Special Rehabilitative Facility (SRF) level of care.

The mission of Unlimited Care Providers is to assist individuals with a Traumatic brain injury in meeting their basic needs, enhancing their quality of life and promoting their independence, while also allowing them to be active participants in decision-making and the direction of services.
